<li>Enter data to filter the search results – for instance, all patients for Dr Xyz would be filtered by only selecting that provider from the drop down.</li>
<li>Select the desired output using the check boxes

<li>Display Matching Data (Patient will duplicate) – if checked, will display patient each time an exam meets the search criteria. For example, if the search is for patients with corneal abrasion and the patient was seen 3 times with an abrasion diagnosis, the search result will display that patient 3 times, rather than just 1 time if the box is unchecked</li>
<li>Include All Demographics – if checked, will display patient demographic information</li>
<li>Search Only Unresolved Diagnoses – if checked will only display patients with an active diagnosis that match search criteria. Resolved or inactive diagnosis will not be displayed</li>
<li>Include Deceased Patients – if checked, will display patients marked as deceased in demographics</li>
<li>Blind Data – data display will hide all identifying factors. Helpful for study participants.</li>

***NOTE – the amount of data being extracted will determine the amount of time it takes for the report to run and export. If a large amount of data is being exported, it is recommended that the user break the report into smaller pieces. For example, if the provider leaves a practice and wants to take all his patient charts with him, the user will want to pull one month of data at a time, rather than several years, so as not to lock the system. This will vary by practice, data, number of visits, etc. Experiment with the export specific to the practice to determine the amount of data can be extracted at once.
